Down Syndrome was formed in 1989 by Hassan (guitar), Fadil (guitar), Medan (bass), Adyll (drums) and Rosli (vocals). At the time, they were one of the earliest punk/skinhead bands in Singapore.

At some point in early 1990, Hassan left the band to form Flow (and is presently the bass player for Bushmen). Also Fadil and Rosli quit the band. Later the same year, 4-Sides was born from the ashes of Down Syndrome. Remaining members Adyll and Medan were joined by guitarist Suhaimi from Stomping Ground (another poineering Singapore HC band) and vocalist Hussein.

In 1992, Medan left the band to start a gothic-influenced band and was replaced by Ah Boon who was with Silent Sorrow, one of Singapore's first Trash Metal outfits at that time. 4-Sides also took part in a compilation titled Lion City Hardcore with many other early hardcore bands from Singapore. The compilation, released to support the "Lion City" hardcore scene, is now one of the most rare and historic releases in Singapore. In 1994, Hussein left the band and was replaced temporarily by Ezar from Voice Out. 4-Sides eventually recruited Imran from Shaolin, Paranorm and Bleeding Gums, to take over vocal duties from Ezar. This current lineup remains unchanged until the present day. In 1999, 4-Sides record their first full-length album called Definition, which was promoted by a performance at Tower Records.
