Information  you need to look for:

Name of author (if available)

Page Title (at the top of the IE window) - leave out if same as URL

Name of Web site (try to figure this out)

Name of publisher or organization associated with the website

RETRIEVED Access date

COPYRIGHT Date

<URL> - the real url, not stuff from a search engine

Remember, you can always do a WHOIS to try to find more information.
